  What is everyone doing for consent forms? I have been using the consent forms on IPS, but they only include TA-1, CJC/Ipa, MOTS-c, 5-amino 1Q, AOD9604. Is anyone willing to share their consent forms? Looking for Tesamorelin and TB4. Is anyone utilizing a general consent form for all their peptides? Or do you have patients fill out a consent form for each individual peptide?

  Leonard, I have them sign a general consent at first stating they are receiving alternative medicine from our clinic which might be outside of the standard of care. I do not offer peptides on the first visit unless they come to me specifically looking for them. (I usually start with sleep and bowels first anyway:). I have a long discussion with them and put in the chart “Patient (and family member) voiced understanding that these are experimental therapies currently with most research in animals (x TA-1, pentosan, liraglutide, tesamorelin…) and some anecdotal evidence in humans. I make no guarantees of outcome nor can I predict any side effects, including death and disability…

  hello all

  After seeing this post, I had our team create SAMPLE peptide consent forms which can be found in the Peptide Therapy: Foundations course – https://ssrpinstitute.org/ssrp-masterminds/peptide-therapy-foundations/

  While the form is based on a consent form from a practitioner, whom I will not name, please have your legal counsel check the form prior to using it as laws differ state to state.

  Again this is a SAMPLE consent form and the SSRP is not liable for any malpractice suit or issue that may arise to which the form does not address as it is only a SAMPLE.
