  Hello all,

  Curious thoughts on testosterone administration, method, and dose for males (topical and pellets for another day, also covered in prior posts here).

  Method: IM or SQ? IM seems the most used, but I like SQ for patient ease – thoughts on it depositing per oil base and adipose?

  Dose: 200mg per 14 days as standard dose – recommendations on daily, every 3rd day, once per week, etc?  Once every 2 weeks seems inappropriate if can be avoided.

  Forms: I’m only really using cypionate.

  If possible I try to recover natural Testosterone production with managing underlying medical/metabolic issues and use of KISSPEPTIN, and or enclomiphene or similar products.  If this is not practical, appropriate or ineffective  I use IM / SQ for those patients requiring replacement Testosterone.  For IM use of a cypionate, or enthanate oil once weekly minimum frequency or twice weekly IM to provide the smoothest coverage.  Also if the patient does not want IM delivery for whatever reason I will take the weekly dose divided by 7  and dispense SQ with excellent results.   Every 2 week dosing is not appropriate considering the 1/2 life of these products.

  Agreed about underlying issues.  Once I start TRT- I use SC injections, typically 2 x per week.  The lower the SHBG the more frequent I dose.  Some do ok on once a week but 2-3 x a week is better (steady state, less peaks and valleys).  I have also had good success with topical application (scrotal application is best)- 100-200mg bid with a Versabase or Atrevis base cream.
