How do I get my practice added to the SSRP Provider Listings map?

You can submit your practice information at any time for review. You can find this page in My Account.

Provider listings are published only when your SSRP certification is up to date. If your certification isn’t current at the time of submission, we’ll keep your details, but the listing will stay inactive and won’t appear on the public map until your certification is current.
