BPC-157 Histamine Response

    Has anyone seen a histamine reaction or mast cell activation with BPC-157? Here is the timeline:

    • 2/5/22: pt reported histamine reaction at injection site, psoriasis outbreak, neuropathy, elevated HR with BPC-175 (claims he injected once on 1/31 and once again on 2/4)
      • I recommended pt discontinue the injection and RTC for evaluation.
    • 2/11: Pt did not RTC but emailed he could not walk due to severe neuropathy from the 2 BPC-157 injections. Claims both lower extremities were numb and painful
      • Recommended pt start a course of TA1
    • 2/14: pt reports his neurologist told him the BPC-157 “triggered small fiber neuropathy” from an immune reaction from the peptide
    • 2/27: pt denies improvement with TA1
    • 4/5: pt reports he is wheelchair stricken since last contacting clinic on 2/27 (still has not returned to the office) and claims he may be admitted to an inpatient program

    I know this is a lot to unpack, but I am unsure how he BPC-157 could be the etiology of this reaction 2 months after he claims to have last injected

    Greg-

    Can ask what it was prescribed to treat in this patient ? What were the baseline diagnoses?

    Also, was this source for the BPC-157 one of our trusted sources?

    And, the patient gave themselves 2 injections…. Was there no negative response with the first one?

    Lastly, are the new neuro deficits unilateral, bilateral/symmetrical, only on the side of the local reaction, etc?

    This seems like a very unexpected response.

    Hi Cynthia,

    • Can ask what it was prescribed to treat in this patient ? What were the baseline diagnoses?
      • He was 4 weeks post L4-L5 fusion at his visit 1/24/22.  Reported lumbar pain and bilateral neuropathy.  TTP at illiac crest BL in office.   +SLR BL but dermatomes were intact

      • Yes, he reported his 1st injection (1/31)caused “histamine reaction and psoriasis flare and rash on abdomen at injection site”.  He did his 2nd injection on 2/4 before reporting the reaction to me

      Lastly, are the new neuro deficits unilateral, bilateral/symmetrical, only on the side of the local reaction, etc?

      • Bilateral and symmetric but reports right side is worse (claimed completes loss of sensation in right foot)

    I assume he can’t walk due to pain and not weakness. If weak and areflexic, guillain barre syndrome needs to be ruled out.

    any recent viral syndromes or vaccinations?GBS could be precipitated by recent surgery as well. It’s alwAys easier to blame the non traditional intervention first

    if EMG and NCV is negative they should have skin punch biopsy to rule out vasculitis and amyloidosis

    Yes, I have had it happen a few times in patients with MCAS and mold.
