Evidence on titration of Semaglutide

    I am wondering if anyone may have any information that can help me out. I have a patient who is asking about the titration of semaglutide. Given Wegovy’s suggestions for the titration of semaglutide I know many of us titrate differently and sometimes faster than that, I am wondering, is there any evidence that titrating up faster is safe?  Is there any precedence or evidence I can offer to her on this rationale?

    Quicker titration can have an increase in potential side effects (nausea, constipation) for some. What is good about compounded versions is you can make micro adjustments as you increase dosing (0.35, 0.75mg) for example. I have increased patients from 0.25mg to 0.5mg in 2 weeks of use. It is nice to check for tolerance first.

     

    Consider adding the low dose leucine from NuBioAge. I have found I can stay in the 1mg – 1.25mg range with good results.

    We use a compounded version at our clinic.  I use a starting dose of 5u and tell my pt’s they may go up by 5u weekly, pending how they feel.  The goal is to have appetite suppression without the negative GI issues.

    Leucine should ABSOLUTELY be added when doing GLP-1 Peptides (among other things). This is the peanut butter to the semaglutide jelly. You can learn all the different strategies in the Weightloss Mastery Course, but specifically Leucine, is an exercise mimetic that lowers the km of the SIRT 1 pathway, so you’re getting faster results.
