Exosome consent form

Hello all, I’m incorporating Kimera exosomes in my practice and looking for a suitable consent form. Are folks using a standard consent for peptides, IVs and exosomes or a specific one for exosomes? Does anyone have one to share?

hi @tjoyeintervenemd-com we use a standard microneedling consent form that you can get from your microneedling provider. we do not use a separate form for exosomes, but i’ll ask Kimera what they say. if you’re doing IV’s just make sure you’ve got an IV consent form for your clinic!
