Help! 61 yo female with ongoing lip and chin angioedma

    The patient reports a history of intermittent tingling, swelling, numbness, and burning sensations of the lower lip and chin. These symptoms had previously improved on a low-histamine diet and adherence to prescribed therapies, but have recently worsened and increased in frequency 5 days after starting Thymosin Alpha 1. She describes new or more severe episodes of swelling and burning of the lower lip and chin, now sometimes involving the entire bottom lip and chin after consuming foods that were previously well-tolerated, such as homemade gluten-free pancakes, apples, baked potatoes, oat bars, and corn chips. She describes her symptoms as burning or tingling, with the swelling lasting for several minutes before subsiding. Recent episodes have increased in severity compared to her prior experiences, now affecting the whole lower lip and chin.

    She denies any introduction of new foods but has recently started thymosin alpha peptide injections (“shots”). The first injection was started last Tuesday, with subsequent doses administered on Tuesday, Thursday, Saturday, and Monday, and she notes her symptoms worsened after these doses. She elected to withhold her planned injection on the day of this encounter out of concern it could be contributing to her symptoms. She continues ongoing supplements, but reported no other new medications.

    The patient is awaiting results of a recent brain MRI performed at the recommendation of neurology, with results still pending as of this visit. She has also been referred to an allergist and already has an appointment scheduled for next week. She denies prior similar food sensitivities before starting her current therapeutic regimen. No reported systemic symptoms such as airway involvement or urticaria.

     

    Current plan:

    • Continue D-hist, HistDAO, GI Resolve, vitamin D3/k2- 5 drops daily with food, magnesium 1-2 tab at bedtime(preferably glycinate at night), vitamin C 1000 mg daily, quercetin, NAC. D-Hist has NAC and quercertin in it. I would like you to have NAC 600 mg twice daily.  I am not sure which one you have at home.
    • Finish alpha-lipoic acid and can add in later as needed.
    • Start Methyl Protect (B-vitamin, methylation supplement) 1 twice daily with food, take before 2 pm daily.
    • Begin thymosin alpha-1 (injectable peptide) as soon as delivered; schedule training with office staff.  TA1 1500 mcg 3 times weekly for two weeks, then 2 times weekly for 2 weeks subq. 2 cycles of this
    • Start BPC/KPV peptide- take 1 orally once daily on an empty stomach for gut healing.  I have a bottle at the office for you.
    • CoQ10 20 mg and B12 2500 mcg injections administered  in clinic
    • Replete vitamin D (with K2) to correct deficiency and promote calcium balance. D3/k2 5 drops daily with food

    I am wondering if the TA1 could have worsened the angioedema?  Any guidance on where to go next?

    Comprehensive food allergy testing is essential; Cyrex Labs assays 2, 4, 5, and 10 are indicated.

    A trial of Larazotide 500 mcg po BID for 60 days, along with Guttides 500 mcg po BID may be very helpful.
