I have a very interesting case, male aged 37 who for the past 10 years gets up in the middle of the night and eats compulsively sugar/ carbs.
He reports that he has had neurology and psych work up in the past, imaging is negative and no real conclusions from psychiatry.
I am waiting on his Ghrelin result but have low Adiponectin of 1.4. Leptin level is only 1.0 His prolactin level is 2.6.
Prior to my consult he was started on Testosterone.
He was also prescribed Semaglutide which has generally suppressed his appetite but not at night. He has worn a CGM and BS does not drop significantly at night. He eats a heathy diet during the day, exercises and maintains a a BMI of 27
I am thinking along the lines of how to increase adiponectin and reduce cravings, but also wondering whether anyone has managed this type of case?
