Mild Anaphylactic Reaction to CJC/Ipamorelin

    I recently had a patient’s wife contact me with the following message.  He is a healthy 62 year old who has been on CJC/ipamorelin since February 2021 for health optimization.

    “… immediate bad reaction to the injection. He turned red all over, itchy, trouble breathing. I gave him a Benadryl, and it subsequently subsided. It was scary and I was prepared to call 911 if his breathing got more difficult. He often experiences itchiness around injection site, but never anything like last night. ”

    How often has anyone heard of this happening?  Should he be off this peptide permanently?  Would you recommend tesamorelin or another GHRH/GHRP?

    I’ve seen some cases of acute urticaria from mild to fairly severe intensity after CJC1295+Ipamorelin.  They responded to oral vitamin C flushes, Quercetin-Bromelain, Alkaselter Gold, Apis mellifica 30C or 200C, and/or oral diphenhydramine.

    I discussed this problem with Linda of Pure Rx, and we decided to try Ipamorelin alone.  It was well tolerated.  A reintroduction of the CJC1295+Ipamorelin elicited hives again, often worse.  The CJC1295 is the culprit.

    You can do a test trial of Tessamorelin and Ipamorelin to see if it’s better tolerated.

    Thanks for the input Mitch.  I’ve seen the mild urticaria and rash but not an anaphylactic type reaction.  I agree with your assessment.

    If you decide to use Tesamorelin, I suggest starting at half the recommended doses.  Tesamorelin is stronger and can cause side effects related to rapid growth and cortisol release in some patients. It is not advised to use it more than 8 weeks at a time, without a break.  Increase dose if tolerated / wanted.   That will also provide some cost savings as Tesamorelin is more expensive than CJC/Ipa.

    I agree completely with Mitch and Abid.  I have seen this a few times in my practice, but luckily it is quite rare. I will usually go back with Tesamorelin alone and see how the pt does. I will go slow with the dosing titration.  From my experience, usually Ipamorelin is the offending agent.  Also, I have been told that patients with a sulfa allergy are more likely to have a reaction with GHRP’s than other patients. Because of the severity of symptoms, I would make sure and have an epipen around in case of reaction.  I think the key is to with a slow titration and proceed with caution with these type of patients.

    Hi All,

    I too have had two patients with anaphylaxis with CJC/Ipa. Turns out it was the pharmacy I was getting them from.

    Changed pharmacy and never got another serious reaction.

    If I do get wheel and flare or pruritus in some patients I give them Pepcid and Claritin and it resolves quickly.

    Thanks, Erika.  I am using California Peptide Pharmacy.  They are new.  It is difficult to get many of the peptides in California.  The vetted mastermind pharmacies don’t ship to California.
