Pediatric post covid anemia

    This patient (17yo female) presents with fatigue, anxiety, depression, mood liability, difficulty with swelling, dizziness with standing. (all of which potentially could be contributed to the anemia) She has failed iron transfusions (no change in ferritin, hgb/hct post infusion), no response to oral supplementation, progesterone supplementation did not affect menstrual flow. Has been started on stimulant and SSRI by her PCP and psychiatric NP to manage symptoms of resistant anemia. Moderate cycle flow but this was preset prior to covid without anemia being present. Is resistant to treatments at this point. (parents divorcing and significant home stress). I would like to specifically target the anemia as I think this is likely the underlying issue though likely mood issues exacerbated by current stress.  Previous labs do not show excessive estrogens. Homocysteine in range (6-8). Significantly low IGF 1 suggesting possible pituitary injury.

    Drawing spike protein quant, genetic data including MTHFR, comt. Gene sight testing to evaluate current SSRI and her tolerance and options if she insists using long term (psychiatric NP wants to add mood stabilizer). Am considering adding SPMs, Plasmalogen Glia, Dihexa/selank, potentially LDN, something to detox spike protein if elevated. Would like to be very selective as she is resistant to taking much at all with her history of multiple provider visits over the last 3 years.  I have seen a similar case with a woman exposed to covid, Signifiant heavy menstrual bleeding, responded to progesterone therapy initially and then ended up with a hysterectomy so she could continue working. Any input would be helpful!

    Gentle exercise daily, Taijichuan, Yoga, etc.

    Consider courses of BPC157, MOTSc,  Synapsin RG3, and Thymulin.
