I started using injectable PPS for my patients with osteoarthritis however many of them complain of brusing and hard lumps at the site for weeks, to the point most of them stopped wanting to inject becuase of the pain. They were injecting SQ.
I am wondering if this is common when injecting?
Also, I always like to confirm, who should not inject PPS?
Tracie I would recommend injecting Pentosan IM. I wouldn’t do it Sub Q. They can do it at home. I can train a person or their partner how to do an IM. It is used for IVF although I wouldn’t use the thigh at home.
People who can come to my office weekly I inject it. If done properly the person doesn’t feel it and there is no bruising
I am finding SQ route works well for my patients – injecting the lateral hip, just above or below the trochanter. Having them firmly massage the injection site for 2 minutes has minimized both the bruising and hard lumps.
Contraindications for this one would be bleeding disorders as it has heparin-like qualities; PUD, variceal disease, those prone to diverticular bleeding, etc. I would not use in someone with a history of HIT or any significant active thrombocytopenia. I would not use in someone with history of intracranial hemorrhage, and those with liver dysfunction will need to be monitored very closely as I have seen transaminitis (mild) with this and also with consideration for reduced clotting factors.
