Peptides in a 47-Year-Old Female With Prior Stroke & Chronic Migraine

    Patient Profile:

    • 47-year-old female

    • History of ischemic stroke at age 40 (no residual deficits)

    • Debilitating migraines since childhood (onset around age 5)

    • History of thyroid cancer

    • S/P total thyroidectomy

    • Currently stable on Synthroid

    • Also S/P appendectomy at age 2

    • No other major comorbidities reported

    Clinical Question:
    Before initiating peptide therapy, I’m looking for guidance on whether there are specific contraindications or cautions when prescribing peptides in patients with:

    • A prior cerebrovascular event (stroke)

    • Long-standing, severe migraines

    I would appreciate any experiences, literature references, or insights related to:

    • Safety considerations for common functional medicine peptides (e.g., BPC-157, TB-500, MOTS-C, AOD, etc.)

    • Any peptides known to exacerbate migraines

    • Any peptides with theoretical risks for increasing clotting, vasoconstriction, or neurologic issues

    • Recommended baseline labs or screening prior to initiation

    this may not be the answer your looking for but you could consider progesterone at night to start, may help with migraines in addition to sleep and energy (no contraindications with cva). i am not familiar with any contraindications to the peptides your suggesting but be aware if stroke recurs the neurologist may point a finger at you! be transparent with your patient do your own research and make an educated decision with your patient.
