Has anyone heard of compounded injectable Semaglutide/ BPC 157/ B6 1.25mg/1.25mg/40mg/ml ? There is a compounding pharmacy promoting this formula with directions: 0.2ml to be injected subq daily for 5 out of 7 days per week for one week (this is equivalent to 0.25mg daily or a total of 1.25mg by day 5). In addition, the directions state: may increase to 0.3ml – 0.4ml subq daily as tolerated. I have not seen any research indicating that semaglutide should be given daily (it has a half life of 7 days). I’m wondering if the thought process is that by adding BPC-157 , gastric side effects will be much less and dose escalation not as crucial?? However, lower doses of Semaglutide (0.25mg and 0.5mg) given once weekly have already been shown to be very effective for weight loss. Also, this pharmacy has explained to practitioners that the injection should be given 30 minutes prior food or 30 minutes after???? The explanation I was told is that it will reduce stomach upset…. I have not heard any information like this before. Any thoughts on this regimen?

February 23, 2022 at 10:18 am #31585I follow the once a week dosing guidelines and have had awesome success with my patients. You are correct, I haven’t seen anything on daily dosing and I bet it can get pricey dosing it like that. Sometimes pharmacies try to create new combinations just to be different. I’d imagine the nausea and other GI stuff would be worse but maybe the BPC helps.

February 23, 2022 at 11:16 am #31587I am not aware of any literature using daily subq dosing of semaglutide. Here is the recommended titration schedule for subQ
Week 1-4 0.25mg weekly
week 5-8 0.5mg weekly
week 9-12 1.0mg weekly
week 13-16 1.7mg weekly
week 17+ 2.4mg weekly.
*In patients who do not tolerate the dosage increase consider delaying the increase an additional 4 weeks*
The only daily dosing I am aware of is oral semaglutide at much higher dosages.
You can take semaglutide with or without food.
Not sure why the BPC is there.

@kelfers35gmail-com I have also seen a lot of success with weight loss at doses of 0.25mg and 0.5mg weekly doses. In my experience, we titrate very few people over 1mg weekly.GLP-1s can be game-changers if done correctly. The key I have seen so far is to start low and go slow. It’s tremendous in what it can do with eating habits and glucose dynamics. If you start too fast people get way too nauseous and lose interest.

My name is David Bonfa and I am one of the Co-Founders of CRE8 Pharmacy. I believe the formula in question is one we have created. Allow me to try to help clear up some confusion with regards to dose and suggested protocols.
*** Keep in mind that the Obesity max dose weekly recommended by Ozempic is 2.4mg once a week (2400mcg’s).Our formula allow doctors to be flexible in their approach.Here is a breakdown in case you want to see all the options you have depending on your stance and medical suggestion:Injection:Semaglutide / BPC-157 / B6 (1.25mg / 1.25mg / 40mg/ml) – 5ml Vial- 0.1ml or 10 units on syringe = 125mcg / 125mcg / 4mg
- This dose will supply you 50 injections per 5ml bottle
- 0.15ml or 15 units on syringe = 187.5mcg / 187.5mcg / 6mg
- This dose will supply you 33 injections per 5ml bottle
- 0.2ml or 20 units on syringe = 250mcg / 250mcg / 8mg
- This dose will supply you 25 injections per bottle
- This CAN be used for a 5 day on, 2 day off protocol but in no way do we recommend protocols. We have simply seen many doctors move to a more daily lower dose approach rather than a higher once a week dose. Please feel free to inject this dose once weekly if you want the patient to receive 250mcg of Semaglutide weekly. That is entirely up to the doctor.
- 0.3ml or 30 units on syringe = 375mcg / 375mcg / 12mg
- This dose will supply you 16 injections per bottle
- This can be used for every other day protocol – again just reiterating that this is what some doctors have been prescribing but please feel free to inject this dose once weekly if you want the patient to receive 375mcg of Semaglutide weekly.
- 0.4ml or 40 units on syringe = 500mcg / 500mcg / 16mg
- This dose will supply you 12 injections
- This can be used for a once a week injection for 3 months
- OR you can use the 0.4ml for a once a week injection for weeks 1-4 and titrate to 0.8ml for the weeks 5-8
With regards to the added BPC-157 – this was included in our formula for assistance to gut repair and especially to help joint pain. The B6 helps with nausea.

We find that many of the patients will have success with the lower doses. I find that its simply all about finding the right dose for most patients. Also adding info Leusynergy from NuBioage has been very helpful as well.

Yes, you are correct! When you get the chance to see what many doctors are prescribing to their patients and getting the desired results you will find the protocol is all over the place. It’s not necessarily a bad thing. At the end of the day I always think its best to start lower and see how they react to it then titrate up after 4 weeks if they can handle it. Have a great day !
