Semaglutide from Tailor Made Compounding (?)

    Dear Dr. Seeds et al.,

    Tailor Made Compounding is now offering injectable Semaglutide.

    I asked their customer support about the dosages and source.  Here is their reply:

    Semaglutide 2mg/ml 1mL – $125.00 when billed to your clinic, and $250.00 when billed to your patient directly.
    Semaglutide 5mg/ml 2mL – $175.00 when billed to your clinic, and $350.00 when billed to your patient directly.
    I [the customer support rep] also consulted with Blake [the chief pharmacist] on our sourcing, and he stated we currently source our Semaglutide from an FDA Certified facility in Pennsylvania!
    My questions are:
    1) Would you trust the quality of the Semaglutide from this pharmacy?
    2) If so, what would be the proper dosing at the concentrations that they offer?

    2. I do imagine we will see other pharmacies making it available so here is the dosing at the most likely concentrations available.

    2mg/ml:

    0.25mg(12.5 units) once weekly for 4 weeks, then increase to 0.5mg(25units) weekly maintenance dose.  May increase to 1mg(50units) after 4 weeks of 0.5mg dose.

    *At 0.5mg dose, all you need for a month’s supply is the 2mg/ml concentration. The BUD is only 30 days so no need to get the higher concentration unless your patient has been titrated up to the 1mg/week dose.

    The recent FDA approval of semaglutide(Weegovy) for weight loss has them titrate all the way up 2.4mg over a long time. Good luck with that I see people having a tough time at 0.5mg.

    I agree with Leonard.

    It depends on the goal.  If it is for glucose management, the low doses work great.  If it is for weight loss, there will be early satiety, nausea, and possibly diarrhea if you titrate it up too fast.  I seen weight loss occurring at all dose ranges.

    Is any other pharmacy compounding Semaglutide that you know of? As anyone had issues with the TM version?

    I have positive experiences with two other pharmacies.  PURE Rx and South Lake compounding carrry it.  South Lake has it in lypholized form.
