A1 51 year old male, BMI=32  presented with severe Right hip OA and is considering stim cells. His xray from 5 years ago showed mild arthritis but now his femoral head is mostly obliterated. He is plethoric, over weight, with poor circulation in his feet and looks like he has sleep apnea (testing pending). Interesting thing is that when he is out of the country, his pain mostly resolves. His gait has been altered by his walking, using a cane. I did a round of PEMF with him over he weekend and it loosened some of the tone up in his ‘functional’ side and his gain improved a bit, so I’m thinking his altered walking and now created a new, harmful gait pattern. Per him, he is fasting, eating and still not loosing weight. His BP used to fluctuate wildly when he’d loos 10 lbs but now can’t seem to loose any weight.

What would be a good weight loss, anti-inflammatory protocol to help heal his hip OA as best as can be expected?

This patient may benefit from the following regimen:

CJC1295+Ipamorelin 0.1 ml SQ BID; cycle 3 months on/1 month off

KPV 500 mcg po BID for 3 months or longer per inflammatory control

AOD+HA 0.5 ml, BPC-157 0.25 ml, Thymosin Beta-4 0.25 ml, and Cerebrolysin 0.5 ml IA into the right hip; reassess in 4-6 weeks; may need repeat injection every 2-3 months

Ketone Esters 10 ml qd initially; may need BID/TID or more depending upon clinical response

Astaxanthol 3 capsules BID; from PGL, (800) 527-9512, order #7876

    Collagen powder 2 scoops BID

    Dr. Fleisher,

Do you have a preferred source for ketone esters?

    Thanks

@hartmanangmail-com great Q, our outcomes for the use of PRP/Bone Marrow Aspirate Concentrate/MicroFragmented Adipose Tissue seem to be worse in the severe Hip OA population. That is not the case for other large joints like the shoulders and knees. Not sure I know the answer why at this point. I probably would have sent to an Ortho who has good outcomes with an anterior approach total hip (most of those patients walk the same day and need relatively little therapy afterwards). I have not been convinced the robot-assisted joint replacements have been superior although anecdotally, some seem to think so.
