    Hi Group, I have a 57yo German female with hypophosphatasia and wonder if anyone has experience using anything other than asfotase alfa for treatment?

    How low is the alk phos?? In general, low alk phos in adults is due to some other underlying issue-biliary, liver? I am sure you made sure D is optimized as well.  If no underlying issues and remainder of the labs are OK, , there is some question in the literature whether there is truly much harm. SO make sure you have dug for etiologies. If still concerned, could try TA1 and a course of ursodiol or tudca.
