I almost forgot, photobiomodulation therapy may be an excellent addition. It is proposed to work at the mitochondrial level with regard to tissue repair. Not an advertisement, however, the “Thor Photobiomodulation” website has lots of free information on PBM’s mechanism of action.

I’m a new member. Peptide therapies seem amazing and as an anesthesiologist it will be interesting to see them applied directly in peripheral nerve sheaths and at the epidural/caudal levels. In the Masterminds 1, an anesthesiologist did say that he injects peptides into nerve sheaths for chronic pain conditions..
Due to the fact that your patient had previous lumbar spinal procedures, as Dr. Seeds mentioned, his lumbar dura mater is definitely disrupted with accompanying dural leaks, etc. If possible, I would consider the following therapies:
- Ultrasound or fluoroscopic guided caudal injection of 20mls Procaine/DMSO (16mls 2% Procaine, 4ml 99% DMSO) and then 20ml ozone gas concentration 30mcg/ml. If you or a provider are familiar with tibial whole bone marrow extraction and injection, it may also be beneficial to add 10ml tibial whole bone marrow mixed with TB-4 (to increase stem cell activity). Please note that ozone/prolozone therapies are effective for both degenerative and CPRS conditions.
- As a follow-up, it may be helpful to repeat the procedure two months later using the procaine/DMSO/ozone as above and adding BPC without the whole blood.
Please note that you may need a larger volume of injectate to reach all areas of the lumbar and sacral plexi. Also, a lumbar epidural procedure will probably will not work due to the dural disruption and inflammation post-surgical inflammation induced scar tissue at the lumbar spine.
