Compounded Semaglutide Losing Efficacy

    Hello,

    I wanted to know if you are seeing cases where semaglutide is not working as well for a given patient as it once did.   One patient with a lifelong struggle of being heavier (52 yof 5’3″ 170 lbs originally, always has exercised regularly, keeps checks/balances on diet, thyroid is regulated/stable) had excellent success with the first usage (helped refine body composition with 30 lbs less fat %), which was over about a 6-9 month time frame like a switch turned on.  She then went off of it for a month maybe a little longer, due to some strange bloating she started to have.  Since going back on it, she has not had the same results with regards to weight loss.  She now requires a much higher dose around 50 units vs. the 25 units that she used to do well with and results are slow or perhaps not even happening.  She could stand to reduce fat % by another 15 lbs.  She swears that the semaglutide has changed, but that’s not the case.

    So, have you seen this and how can I help her break through?  Has she acclimated?  Over saturated receptors?  Metabolic damage?  Can you help me understand why this happens and what can be done to correct it?

    I’m including some strategies that I have found to be successful with the people I work with. I will preface this by saying there are people far smarter than me in this organization, and I am privileged to have the chance to learn from them! I occupy a unique space in the industry. My background is strength coaching. I didn’t have the stomach for medical school and I am dyslexic. I have a passion for learning and helping people which led me to the place I am currently at. In addition to training clients a consult with physicians sharing the knowledge I’ve learned from courses and let them handle the medical side of things. It has been a wonderful experience and allowed me to put my clients in the hands of capable doctors and given them a footsoldier to keep an eye on their patient and relay any positive or negative changes in health which ultimately makes their office more efficient and informed.

    Through over 50 consultations, I’ve rarely had to increase the starting dosage for peptides. Here’s my approach:

    1. Baseline Measurements: I always start by assessing both fasting and post-glucose levels. This helps me monitor improvements in blood sugar management.

    2. Dosage Adjustments:

       – Starting Point: I typically start with 0.25 mg of Semaglutide.

       – Monitoring: If blood sugars range from 120 to 130, I look for improvement towards the target range of 80 to 90.

       – Titration: If numbers don’t move, I consider increasing the dosage.

    Introducing Retatrutide

    If Semaglutide’s effect plateaus, I might introduce Retatrutide due to its glucagon modulation, which can boost energy expenditure. After a short two-week exposure, I often return to Semaglutide.

    Alternative for Lipid Management

    For managing lipids, I prefer Tirzepatide over Semaglutide, considering budget constraints.

    Creating a Supportive Environment

    Success with peptides goes beyond dosage:

    – Hydration: Proper hydration is crucial for cellular signaling. I recommend iCell Water from NuBioage, combined with 2 g of taurine for its benefits on insulin sensitivity and CNS recovery.

    – Protein Intake: Protein increases satiety a can enhance the thermic effect of food by up to 30%. It also preserves muscle mass, crucial for resting and exercise calorie burn.

    – Exercise: Encourage clients to engage in strength training 2-3 days a week and daily steps. Muscle is our currency of aging!

    Digestive Health

    Digestive health is key for optimal results:

    – Lactulose: Decreases inflammation and supports prebiotic activity.

    – Fiber: Incorporate resistant starch (green banana starch)for increased butyrate production, Sunfiber for its ability to bind to and help remove endotoxin and lipopolysaccharide, and Pectasol for removing metals for gut health and detoxification.

    Enhancing Cellular Function

    – Breathing Exercises: Techniques like box breathing can help achieve a parasympathetic state.

    – 5-10-15-20 Method: Before meals, this involves 1 min of bodyweight squats to increase GLUT 4 translocation, 5 deep belly breaths to switch to a parasympathetic rest and digest state, mindful chewing 10 seconds per bite to support release of salivary amalyse, 15 seconds between bites to prevent rushing and becoming sympathetic (sidenote: cortisol can dump sugar into the bloodstream and change the normal response to food in some of the athletes I work with depending on their nervous system state they can have completely different blood sugar reactions to the same food and same amounts) and post-meal movement to enhance digestion and nutrient absorption for 20 minutes following the meal make it a point to move around. This can be a walk outdoors or for people who have a sedentary job park farther away from the building choose the stairs and stand up every time you take a phone call.

    Optional Supplement Support

    – **TUDCA**: 500 mg/day to support bile production and detoxification.

    – Peptides: Use MOTSc and/or SS31 for mitochondrial health, combined with methylene blue, red light exposure, and sauna for heat shock proteins.

    – BPC-157: For digestive support and hormone receptor density.

    – Tesamorelin: For growth hormone release and lipolysis.

    – 1mna: To optimize NAD+, support energy production, and improve fat and glucose metabolism.

    This holistic approach ensures clients build health sustainably. It’s about playing the long game, not just quick weight loss. I hope these insights help you as much as they’ve helped my clients.
